Theft and Counterfeiting of Serialized Prescription Blanks
Healthcare facilites in New York State have seen a significant increase in the theft of Official

New York State Serialized Prescription Blanks. These prescriptions
are being counterfeited and presented to pharmacies primarily for
oxycodone 30mg tablets.
